Appointment system for GP and Advanced Practitioner Appointments
As demand increases it is more important than ever that we allocate appointments based on clinical need.Therefore all appointment requests from patients to see a GP or Advanced Practitioner, will now be made via completing a patient triage form, this form is be available for patient to complete themselves via our website/NHS app or from our telephone options. If patients are unable to complete the form themselves, this can be completed by our reception staff if patients choose to contact the surgery via telephone or by attending the surgery in person.
All completed patient triage forms will be reviewed by our clinical team withing 2 working days. The team will determine:
- The right person / service is to deal with your concerns
- The right timescales in which you need to be seen
- The right mode in which you need to be seen i.e. telephone / face to face
Urgent Appointment
Urgent appointments are traiged by submitting our patient traige form via our webiste, option 2 on our telephone welcome message or via tyhe NHS app. You can also contact the surgery in the numbers below and speak with the reception team

Routine Appointment
To request a routine appointment:
- use our Patient Traige form on our website home page or via the NHS App

- visit the surgery and speak with a receptionist, Monday to Friday from 8am to 6:30pm
Please note that a routine appointment is for 10 minutes duration and is for one person and one medical condition. If you feel you may need longer please book a double appointment.
Patients arriving more than 10 minutes late will not be seen and will have to make another appointment.
Home visits
If you are housebound and need an appointment, we will do a home visit. We will phone you first to understand what you need.
To request a home visit, it’s helpful if you phone the practice before 10am as possible.
Extended hours
You can now book evening and weekend appointments at 4 hubs based across Rotherham
The appointments are routine which means they must be booked in advance and are not for urgent problems. You can book an appointment by contacting your usual GP Practice where you are registered, they will have access to all the available appointments .
The hubs will have appointments with any one or more of the following clinicians. GP, Advanced Nurse Practitioner, Practice Nurse and Health Care Assistant. The receptionist at your GP Practice will help to direct you to the right appointment type.
The Hub Surgeries are held at the following times and locations:

- Saturday Mornings (8am – 1.00pm)
- Broom Lane Surgery – (Broom Ln, Rotherham S60 3EW)
- Weekday Evenings (6.30pm – 8pm)
- Saturday Mornings (8am – 12 noon)
- Sunday Mornings (8am – 12 noon)
- Kimberworth Park Surgery – (Langdon Rd, Rotherham S61 3QH)
- Saturday Mornings (8am – 12 noon)
- Dalton Surgery – (Saville St, Dalton, Rotherham S65 3HD)
- Monday Evenings (6.30pm – 8pm)
- Saturday Mornings (9am – 1pm)

However you choose to contact us, we may offer you a consultation:
- by phone
- face to face at the surgery
- on a video call
- by text or email
Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.
Cancelling or changing an appointment
If you cannot attend an appointment for any reason please inform us as soon as possible in order for us to give the slot to someone else.

If you need help when we are closed
If you need medical help now, use NHS 111 online or Call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
